Starr Regional Medical Center is currently seeking talent to fill the position of Discharge/Admission Nurse (RN) for our Administrative Department. This job is permanent, PRN.

*Core competencies include, but are not limited to the following:*

* Documents patient care given.

* Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.

* Collaborates as needed across disciplines to coordinate patient care, including admission, patient transfer, discharge, referral and spiritual/psychosocial support needs.

* Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

* Works as an advocate for the physical and emotional well-being of the patient.

* Establishes and maintains efficient methods of ensuring the medical necessity and appropriateness of all hospital admissions.

* Performs admission assessments and required documentation on ER direct admits, surgery, and inpatient.

* Consults with nursing staff and multidisciplinary team regularly to evaluate patient’s status and appropriateness of medical care, including admissions, length of stay, transfer, and discharge.

* Monitor’s patient and family satisfaction.

* Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.

* Performs chart reviews to identify actual or potential issues with service delivery, patient outcomes, and satisfaction, compliance, cost, and reimbursement.

*Minimum Education:*
Associate’s degree in Nursing:

Bachelor’s degree in Nursing:
X Preferred â–¡ Required
*Required Licenses:*
[Tennessee, United States] Registered Nurse
Current RN License in the State of TN or another compact state

Certifications:
The below certifications will be required at the time of hire:
X Basic Life Support (BLS)

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Must achieve a passing score of 85% on the Medication Administration Exam
