What You Will Do:
You will have the ultimate responsibility to help implement customized care plans that encompass solutions to nurture the whole person in mind, body and spirit. You will be providing impeccable care to clients by assisting in scheduled activities as well as overall care management. You will treat your clients like family, going above and beyond to enhance their quality of life.
Responsibilities
- Assist independent living residents in coordinating physician care and physician visits
- Per Resident and/or POA’s instruction, necessary personal and demographic info will be provided to the Residents’ designated physician or other health care provider offices in order for resident to obtain health care
- Assist residents in the coordination of home health care services
- Complete incident reports
- Answer all Aerial pendant calls for all Independent Living residents and maintaining daily daily reports
- Fax resident prescriptions to pharmacy
- Perform Blood Pressure Clinics upon request and assist with weigh-in and logging of blood pressure, pulse, and weigh in results
- Give health education presentations to the Residents upon request
Qualifications
-LPN ( Licensed Practical Nurse) or RN ( Registered Nurse)
– Previous experience in a similar role is preferred
– Able to pass a background check
– Knowledge of Home Care preferred
– Strong interpersonal skills with the ability to build rapport with clients
– Excellent communication skills, both verbal and written
– Ability to work independently as well as part of a team
– Strong organizational skills with attention to detail
– Proficient in using computer software for documentation and data analysis
– Authorized to work in the US.
